Schedule: fulltime, Monday - Friday

The Clinical Social Worker is responsible for assuming assigned patient caseloads on inpatient medicine and surgery. Reporting to the Social Work Manager or a designated supervisor, the social worker will provide bio-psychosocial assessments and interventions for patients and/or families impacted by a patient's acute medical needs and any psychosocial factors or barriers impacting their care. The social worker will identify high risk psychosocial factors impacting a patient's health status and, as an integral member of the interdisciplinary team, will help the hospital staff understand the influence of those factors on the patient's course of medical care. The social worker will effectively intervene through evaluation, emotional support, provision of resources and recommendations, and team collaboration around potential barriers to care in the community and around discharge needs. The social worker will have knowledge in mandated reporting, cultural and/or age specific impact, homelessness, substance use disorders, psychiatric illness, and the various community agency and resources available. The social worker will participate in departmental and hospital-wide committees as assigned and will collaborate with other disciplines in the hospital. Mentoring and supervision of social work students will be expected if license appropriate.

  • Provides bio-psychosocial assessment of patients evaluating for coping style, understanding of illness/adjustment/compliance, barriers to care, cultural issues or age-related issues impacting the care, abuse/neglect, intimate partner violence, community violence, trauma, substance use disorders, unstable housing or homelessness, food insecurity, psychiatric disorders, and other factors impacting care.
  • Meets with families of patients when indicated to assess family relationships/systems as they relate to the care of the patient. Identifies family decision makers, caregivers, and evaluates their understanding of the patient's medical issues and needs. Evaluates family coping style as well.
  • Participates as member of interdisciplinary team in daily rounds on assigned unit.
  • Employs a wide range of clinical interventions such as individual or family interventions using psychoeducation, emotional supportive therapy or other therapeutic modalities such as CBT or motivational interviewing, and other counseling strategies as indicated.
  • Assesses for and files mandated reports as indicated for child abuse/neglect, elder abuse/neglect, disabled persons abuse/neglect, and institutional abuse/neglect. Collaborates with team around same.
  • Provides substance use assessment and assists with treatment options, referrals, and resources based on patient preference.
  • Works with patients and families around advanced directives including assisting patients with completion of health care proxies, helping determine if copy of HCP exists elsewhere and how to obtain, and working with team and legal counsel should guardianship be necessitated.
  • Assist with coordination of family/team meetings and provide guidance during meeting as indicated.
  • Facilitates appropriate use of hospital and community resources as indicated.
  • Will participate in clinical supervision with Social Work Manager or designated supervisor.
  • Will participate on hospital wide committees as opportunities present for Social Work involvement.
  • Will adhere to the NASW Code of Ethics for social work profession and practice within the scope of social work licensure.

Qualifications
  • Master's Degree in Social Work from an accredited program required.
  • Current professional licensure in Massachusetts required.
  • LCSW required, LICSW preferred
  • Prior experience in a medical setting preferred.
  • One to two years minimum Social Work experience preferred, internship/field placements experience considered if related to position skills required
  • Sound clinical judgment and ability to set priorities in management of caseload.
  • Ability to guide and provide leadership when indicated to an interdisciplinary group.
  • Knowledge base of the psychosocial assessment, Case Management, Social Work, community resources and related regulations.
  • Excellent interpersonal skills including ability to collaborate and negotiate both within the hospital setting and with community contacts, tolerance for challenging cases, and patience.
  • Computer skills and ability to document in an electronic medical record
  • Fast paced setting working at Acute Medical Hospital on inpatient medical/surgical floors and in the emergency department if needed.
