The Neuro-immunology Center Nurse Practitioner will be responsible for:

Joining a growing team committed to the care of patients diagnosed with neuro-immunological conditions, ages infancy to young adult.
Partnering with, Neurologists, Nurses, Administrative Staff and other pediatric subspecialists to provide care across a variety of settings including inpatient, infusion center and ambulatory.
Enhancing continuity of care, facilitating and coordinating a comprehensive plan of care, engaging in frequent contact with patients and families to provide anticipatory guidance for their care needs across the continuum through phone calls, virtual visits, in person visits and/or collaborating with other team members.
With neuro-immunology attending guidance, evaluating and managing outpatients in neuro-immunology clinic, including patients with new symptoms, patients with chronic disease management needs, and patients transitioning to adult neurology care.
Facilitating care for neuro-immunology patients receiving outpatient infusions in the Boston and Waltham infusion centers which may include writing orders, seeing patients during infusions and/or collaborating with patients, families, infusion center nursing and administrative staff.
Providing continuity of care and facilitating transition to outpatient setting for inpatient neuro-immunology consult patients along with the neuro-immunology fellows and attendings.
Attending biweekly multidisciplinary neuro-immunology center meetings.
Working 40 hours per week.

To qualify, you must have
* A Master's degree in Nursing
* A current MA NP license (FNP or PNP)
* 5+ years of pediatric RN experience
* 2-3 years of Neuroscience nursing experience preferred
* 2-3 years of Advanced Practice experience assisting in the care coordination of patients with complex medical conditions, strongly preferred
* Professional certification preferred or willingness to pursue (CNRN)

**Sign-on bonus of $5,000.00 for full-time (40 hours per week) NP hires (who have not worked at BCH in the past 2 years.)
