Responsibilities:

  • Provides support and guidance to adults experiencing crisis.
  • Dispatch into the community with a clinician to support AMCI clinicians as well as YMCI clinicians as needed.
  • Assists in the development of a risk management/safety plan.
  • Refers clients to support services.
  • Acts as a bridge between the client, agencies, and community supports.

Qualifications:

  • Grade C04: Case Manager II: Requires a Bachelor's degree; in Psychology, Counseling, or Social Work with 1 year experience working in the field.
  • Bilingual/Spanish highly desired.
  • Knowledge of substance use disorder, mental health and related issues, familiarity with community resources and entitlements.
  • Strong analytical skills, including the ability to recognize problems, think creatively, and devise innovative solutions.
